Title V Forms, Applications, and Guidance

Local educational agencies (LEAs) must apply annually to get federal Title V, Part A, funds. The application for Title V, Part A, funds is part of the Elementary and Secondary Education Act (ESEA) consolidated application that LEAs file to get all the funds to which they are entitled. (Note: The ESEA consolidated application is completed almost entirely on the Internet but does include some paper forms that must be signed and submitted.)

LEAs receiving ESEA funds are required to complete a year-end report accessed through the ESEA application.Aid Claim
