Special Education Aide License Requirements

To apply for or renew licensure as a Special Education Program Aide (883), submit the items below to the Educator Licensing Team's Milwaukee Mailing address shown on the License Application Form Instructions. Applicants must be 18 years of age.

1. A PI-1602-PA license application form (downloadable), including a completed, notarized Conduct and Competency review form.
2. For initial license application only: Section II of the PI-1602-PA application form must be signed by the principal/administrator of the school district or CESA that is hiring you.
3. $75 payment for an Initial 5-year License. $75 payment for Renewal of a 5-year license.
4. Fingerprint Cards if required.
5. Mail all application materials to the Milwaukee Bank address below.
