Requirement

Example of Documentation

Identify and diagnose K-4 students who have difficulty learning to read.

List of all students, noting those who have difficulty learning to read.

Copies of informal and formal assessments used.

Have identification and diagnosis be directed by a person who has earned at least six semester credits in courses dealing with the measurement of reading skills and achievement or the diagnosis of reading difficulties or be directed by the distsrict reading specialist required under s. 118.015, Stats. Name and certification of staff person directing identification and diagnosis.
Provide instruction directed toward removal of deficiencies. Record of the names of children who participated, noting the evidence of progress and when services were completed.
Coordinate instruction with all other reading instruction. A copy of the plan of instruction and services.
Develop a written plan to improve reading performance if fewer than 80% of pupils in the district or in a school within the distsrict score above the Minimal proficiency level on the test administered under 2.121.02(1)(r), Stats. A copy of the plan developed to improve reading performance.
